When submitting a privacy or compliance request, please include your full name, contact information, the nature of your request, and enough information for WeHealth to identify the relevant account, program, provider-supported workflow, consumer app feature, or communication. We may need to verify your identity before responding to certain requests.
3. SECURITY INCIDENT REPORTING
If you believe your WeHealth account, login credentials, device, patient information, PHI, consumer health information, or other sensitive information may have been accessed or used without authorization, notify WeHealth promptly using the contact information above. Please do not include passwords, full Social Security numbers, full payment card numbers, or unnecessary sensitive information in an unsecured email.
4. PROVIDER-SUPPORTED HEALTHCARE PROGRAMS
If your inquiry relates to medical care, treatment, prescriptions, care instructions, billing by a healthcare provider, or your official medical record, you should contact your treating healthcare provider, medical group, clinic, health plan, or healthcare organization directly. WeHealth is a technology company and is not the treating provider, EMR/EHR, pharmacy, or prescribing platform.
If WeHealth receives a privacy rights request or medical-record-related request involving PHI maintained on behalf of a covered entity, WeHealth may direct the request to the applicable provider, medical group, health plan, clinic, or covered entity, or may assist that covered entity as required by applicable law and agreement.

Unless a separate written agreement with WeHealth states otherwise, formal legal notices to WeHealth should be sent by certified mail or nationally recognized courier to:

Formal legal notices should be sent by certified mail or nationally recognized courier. Email notices may be accepted for operational communications, but email may not constitute formal legal notice under applicable agreements unless expressly stated in the applicable agreement or required by law.

You may submit complaints, accessibility concerns, privacy concerns, security concerns, or general feedback using the contact information above.
